    Welcome to the Zoi family!

    Thank you for purchasing the Zoi Family OPD card. This card is valid only for the person/family for whom it is issued and can be used for outpatient consultations with specified doctors in specified specialties at Zoi Hospital Ameerpet, Hyderabad. To book your first consultation, you need to visit our front office at our Ameerpet branch, present your card - within 30 days of its purchase, and fill out a registration form so that your card is mapped against your name. You and your family can now use this card for up to 120 days from the date of purchase or until six visits are completed - whichever is earlier.

    TERMS AND CONDITIONS

    What does it cover: You are entitled to a total of six consultations within 120 days from the date of the purchase of this card. Six members of your family (including yourself) whose names you mentioned in the registration form at the time of activating this card can come and consult our doctors. To use this card, please present it to our front office staff. You/they can choose to consult doctors in any of these specialties:
    • Gynecology / Obstetrics
    • Pediatrics
    • Orthopedics
    • Internal medicine
    • General Surgery
    • Physiotherapy
    You can consult any doctor any number of times, subject to the total cumulative number of visits by all family members being six, and they must have occurred within 120 days of the purchase of this card. This card is valid for outpatient consultations by PRIOR appointment at Zoi Hospital, Ameerpet, Hyderabad only. To book an appointment, please call our telesupport team at 040 47 201 201 between 8 a.m. and 8 p.m. on weekdays. After a consultation is completed, one visit box is checked off. Once all six boxes have been checked off, the card expires.

    Exclusions:

    • This card cannot be used for online appointments, either on our website or through any aggregators. It can only be used to book a consultation via our call center.
    • Unused visits will lapse and will not be reimbursed.
    • Mutilated cards will neither be accepted nor replaced.
    • Disputes, if any, will fall under the jurisdiction of Hyderabad Civil Courts
    • Zoi Health Pvt Ltd. reserves the right to make changes / withdraw this offer at any time that they wish.
